If you’re a high‑initiative Registered Nurse who loves autonomy, clinical complexity, and being the go-to expert for a team, this role puts you at the center of meaningful change. This isn’t traditional home care. This is clinical leadership , health strategy , and hands‑on nursing excellence rolled into one. Fully staffed homes with strong DSP & Manager support Small geographic radius — minimal driving Stable caseload you’ll get to know well Collaborative team: medical, behavioral, and administrative support This is steady, proactive nursing focused on wellness — not crisis response. What You’ll Do You’ll support 6–10 community homes (about 25–35 individuals) by providing routine health oversight: Daily wellness checks during home visits Monitoring chronic conditions Reviewing medications & MARs Coaching staff on simple health tasks Preparing individuals/staff for medical appointments Documenting health updates Communicating with families, providers, and internal teams This is field‑based community nursing with a consistent rhythm . Health monitoring & early‑change detection Medication oversight & reconciliation Appointment coordination & follow‑through Staff training on practical health skills Collaboration with Behavioral Services Documentation & quality tracking What You Bring Active NJ RN license ADN or BSN 2+ years of nursing experience (community, home care, residential, or IDD a plus) Comfort working independently in the field Valid driver’s license & reliable transportation The Impact You’ll help adults with intellectual and developmental disabilities stay healthy, safe, and comfortable in the homes they love — while enjoying a calm, supportive nursing environment with predictable days and meaningful relationships.
