Speech-Language Pathologist (SLP) - Part-Time
Location: Soldiers Grove, WI
Facility: Soldiers Grove Health Services
Setting: Skilled Nursing Facility (SNF)
Continuum Therapy Partners is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team at Soldiers Grove Health Services in Soldiers Grove, Wisconsin.
This part-time Speech-Language Pathologist opportunity offers the chance to provide exceptional speech therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.
As a Speech-Language Pathologist (SLP) , you will evaluate and treat adult and geriatric patients with communication, cognitive, speech, language, and swallowing needs while collaborating with an interdisciplinary rehabilitation team focused on quality patient outcomes.
Responsibilities- Evaluate patients and develop individualized speech therapy treatment plans.
- Provide skilled speech-language pathology services in a skilled nursing facility setting.
- Treat patients with communication, cognitive, speech, language, and swallowing disorders.
- Complete swallowing evaluations and provide appropriate interventions as needed.
- Develop treatment strategies to improve patient safety, communication, and quality of life.
- Monitor patient progress and adjust treatment plans based on patient needs.
- Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
- Complete accurate and timely documentation according to company, state, and federal guidelines.
- Educate patients, families, and caregivers on strategies to support communication and swallowing safety.
- Graduate of an accredited Speech-Language Pathology program.
- Current Wisconsin Speech-Language Pathologist (SLP) license required.
- Current ASHA certification required.
- Skilled nursing or rehabilitation experience preferred but not required.
- New graduates are encouraged to apply.
- Strong communication, organization, and teamwork skills.
