Demonstrates clinically sound decision making skills.ResponsibilitiesResolves drug distribution problems assuring accurate and timely dose delivery to all patientsInterprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans.Problem Identification & ResolutionProvides drug information to nurses, MDs, and other health care professionalsEducational ActivitiesDocuments medication use variances (MUVs) and Adverse Drug Reactions (ADRs).Regulatory and Safety Quality ComplianceQualificationsEducationGraduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.ExperienceTwo years experience in an acute care hospital pharmacy is preferred for this position.LicensureLicensed to practice pharmacy in Connecticut (or eligible for reciprocation)Special SkillsThis individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management.
