Social Worker Case Manager

Job Overview Inova Fair Oaks Hospital is looking for a dedicated Social Worker Case Manager to join the Case Management Team. This role is PRN Day Shift: average of 2-3 shifts per month. Orientation will be 5 days per week for approximately 6-12 weeks. Inova Fair Oaks is a top-ranked 174 bed acute care community hospital serving the rapidly growing suburbs of Northern Virginia. Responsibilities Participates in assessment of patients’ biopsychosocial needs through review of patient information, personal contact with patients/families, and interdisciplinary team members. Communicates routinely with patients, families, interdisciplinary team, and appropriate parties regarding patients’ care plans and progress toward treatment goals. Ensures all transition options are communicated to patients/families and team members, documented for continuity of care, and refers cases appropriately to resolve barriers. Assesses psychosocial risk factors based on preliminary risk screenings, evaluating prior functional levels, support systems, reactions to illness, and coping ability. Intervenes with patients/families regarding emotional, social, and financial consequences of illness or disability. Provides counseling and interventions related to treatment and end-of-life decisions and advocates for patient/family empowerment to make autonomous healthcare decisions. Provides discharge planning and continuity of care for assigned patients in acute and post-acute settings. Initiates and facilitates referrals to clinics, home health care, hospice, SNF, acute rehab, LTAC, TCM, medical equipment and supplies as indicated. Collaborates with the interdisciplinary team, patients, and families to coordinate discharge planning and post-discharge services and transition of patients from hospital to their discharge setting. Minimum Requirements Education: Master’s Degree in Social Work. Experience: Minimum of 1 year of experience in clinical care or clinical case management. Certification: Basic Life Support (BLS) for Healthcare Provider certification from the American Heart Association, required upon start. Preferred Qualifications 1 year of previous inpatient case management experience and discharge planning experience is highly preferred. Ability to complete onsite orientation/training scheduled Monday-Friday (5 days per week) for approximately 6-12 weeks before PRN schedule begins.
