• Coordinate patient appointments and follow-up consultations to ensure continuity of care.

• Build and maintain strong relationships with healthcare providers and community organizations.
• Document patient interactions and maintain accurate records in compliance with HIPAA regulations.
• Collaborate with internal teams to enhance patient care processes and service delivery.
• Conduct outreach initiatives to engage and inform potential patients about services offered.
• Stay updated on industry trends and best practices to continually improve patient experiences.

Requirements:

• Proven experience in patient care or healthcare liaison roles; Cardiology is a plus.
• Strong communication and interpersonal skills to foster relationships.
• Knowledge of Cardiology and patient care needs.
• Ability to work independently and as part of a collaborative team.
• Proficiency in using electronic health records and patient management systems.
• Compassionate demeanor with a genuine interest in patient welfare.
• Relevant certifications in healthcare
