Director of Patient Care Services

Job Summary:

We are seeking an experienced and compassionate Director of Patient Care Services to join our team at Anchor Health in Sacramento, CA. The Director of Patient Care Services will play a crucial role in leading our patient care services department, ensuring the delivery of high-quality care, and driving operational excellence.

Responsibilities:

• Lead the patient care services department, overseeing the development, implementation, and evaluation of patient care strategies and initiatives
• Collaborate with cross-functional teams to develop and maintain patient-centered care standards and guidelines
• Foster a culture of excellence, quality, and safety, aligning with Anchor Health's mission and values
• Analyze and report on patient care outcomes, identifying opportunities for improvement and implementing corrective actions
• Serve as a liaison between clinical and non-clinical staff, ensuring seamless communication and collaboration
• Participate in strategic planning, identifying opportunities for growth and expansion in patient care services
• Develop and maintain relationships with healthcare providers, referrals sources, and internal stakeholders
• Ensure compliance with regulatory requirements, accreditation standards, and industry best practices

Requirements:

• 2+ years of experience in a leadership role in patient care services or a related field
• Strong understanding of patient care delivery models, care coordination, and care transitions
• Excellent communication, collaboration, and leadership skills
• Ability to analyze data, identify trends, and make informed decisions
• Experience with budgeting, forecasting, and financial planning
• Knowledge of healthcare regulations, accreditation standards, and industry best practices
• Bachelor's degree in Nursing, Healthcare Administration, or a related field (or equivalent experience)
• Experience with HomeCare Homebase EMR
