Description

• Assess patient needs in an inpatient or outpatient environment and formulate personalized treatment plans
• Implement physical therapy techniques to enhance mobility, strength, balance, and overall function
• Engage with a team of nurses, physicians, case managers, and rehabilitation professionals to maximize patient outcomes
• Provide education to patients and families regarding therapeutic objectives, discharge processes, and home exercise routines
• Document patient evaluations, treatment methods, and progress as per facility and regulatory standards
• Participate in care team discussions and interdisciplinary meetings to ensure seamless patient care

Education

• An accredited Doctorate in Physical Therapy (DPT) or a comparable degree is required

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is desirable
• Specialized certifications (e.g., neurologic, geriatric) are advantageous but not mandatory

Skills

• Preferred previous work experience

• Excellent communication and collaborative skills
