Patient Care Coordinator Nurse Navigator

LifeBridge Health, Inc. seeks a Nurse Navigator to guide patients and families through complex care pathways across our regional health system. The Nurse Navigator coordinates appointments, diagnostics, referrals, and transitions of care, serving as a primary point of contact and advocate. This role provides education, symptom management guidance, and emotional support, collaborating closely with physicians, social workers, and allied health teams. The Nurse Navigator tracks outcomes, removes barriers to care, supports quality and safety initiatives, and promotes culturally competent, patient‑centered care in alignment with our community-focused mission.
