What You'll Do Provide evening hospice visits to patients throughout the Twin Cities Metro. Perform assessments, symptom management, medication administration, and comfort-focused nursing care. Educate and support patients, families, and caregivers. Deliver care in homes, assisted living communities, memory care, and skilled nursing facilities. Coordinate closely with physicians and the interdisciplinary hospice team. Complete timely and accurate EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience is welcome, but nurses from ICU, ER, oncology, med-surg, home health, telemetry, step-down, and palliative care are encouraged to apply. Strong clinical assessment and communication skills. Confidence working independently in the field. A passion for building meaningful relationships through exceptional patient care.
