Hospice Admissions RN: Patient Care & Coordination

Halifax Health ExpressCare is seeking a Hospice Admissions RN to manage hospice referrals within hospital settings and other locations. You will perform patient assessments, admit eligible patients, educate families, coordinate transfers, and ensure pain management while supporting staff. Florida RN license and valid driver’s license required; hospital experience preferred.
