Registered Nurse (RN) – Home Care Assessment & Care Plan Development

Location: Burlington, Camden, and Gloucester Counties, NJ
Company: SYNERGY HomeCare of Mt. Laurel
Job Type: Part-Time / Per Diem / Contract
Compensation: $40–$75 per visit, depending on experience and travel

Job Summary

SYNERGY HomeCare of Mt. Laurel is seeking a compassionate and detail-oriented Registered Nurse (RN) to conduct non-medical client assessments and develop individualized care plans for clients receiving home care services.

This role is ideal for an RN who enjoys working independently, building relationships with clients and families, and helping ensure that each client receives safe, personalized, and appropriate care in the home setting.

Responsibilities

• Conduct in-home assessments for prospective and current clients.

• Evaluate physical, cognitive, emotional, environmental, and safety needs.

• Identify fall risks, mobility concerns, and activities of daily living (ADL) support requirements.

• Develop or update individualized care plans based on client needs and goals.

• Review medications and health conditions to support safe caregiving practices.

• Collaborate with clients, family members, caregivers, physicians, hospice providers, and other healthcare professionals as needed.

• Conduct supervisory visits and periodic reassessments.

• Provide caregiver guidance and recommendations related to the care plan.

• Ensure compliance with New Jersey regulations and agency policies.

• Complete accurate documentation and assessment reports within established timeframes.

Qualifications

• Active New Jersey Registered Nurse (RN) license in good standing.

• Minimum of one year of nursing experience preferred.

• Home care, hospice, geriatrics, case management, or community health experience preferred.

• Strong assessment, documentation, and communication skills.

• Ability to work independently and manage a flexible schedule.

• Reliable transportation and willingness to travel within the service area.

Preferred Experience

• Home Care Service Firm (HCSF) experience.

• Dementia and Alzheimer's care.

• Hospice and palliative care.

• Fall prevention and home safety assessments.

• Care plan development and caregiver supervision.

• Excellent for staying active in retirement.
