Our organization is looking for Per Diem physicians specializing in adult and senior care to enhance our expanding post-acute care team in the Cumberland area. This role is vital in delivering high-quality, compassionate services to our elderly population. The post-acute care team operates across various skilled nursing facilities throughout the county.

We encourage applications from primary care physicians, hospitalists, and those with geriatric training.

Position Overview:

• Deliver direct patient care across a range of facilities, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
• Collaborate with facility leadership, hospital discharge teams, care management, and other specialties to ensure optimal quality care aligned with shared objectives.
• Engage in diverse healthcare fields within post-acute care, spanning general primary care, internal medicine, geriatrics, and acute rehabilitation services. Our clinicians work alongside various specialists, enhancing their knowledge and skill sets.
• Supervise and collaborate with Advanced Practice Providers.
• Utilize EPIC EMR, along with online diagnostics such as PACS imaging and lab results.
• Participate in clinical rounds Monday through Friday, with appealing daily schedules and flexible models. Overnight and weekend on-call duties are limited to phone coverage, shared equitably among partners and APPs, with favorable ratios.

Professional Qualifications:

• Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• A team player capable of delivering personalized patient care and demonstrating positive clinical outcomes.
• A strong commitment to providing high-quality, compassionate care to patients.
• Eligible for licensure in Pennsylvania.
