Physical Therapist - Home Health and Hospice

Location: Kenmare, ND

Assignment Duration: This is a 13-week contract position.

Schedule: 5 days/week, 40 hours/week

Job Description: The Physical Therapist provides specialized physical therapy services within the home health and hospice environment, delivering comprehensive, patient-centered care while ensuring accuracy, safety, and compliance with all regulatory standards. This role provides rehabilitative services to patients, focusing on treatment, rehabilitation, safety, maintenance, and education, particularly for the geriatric population.

Responsibilities:

• Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans.
• Supervise and coordinate care with Physical Therapist Assistants (PTAs).
• Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence.
• Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning.
• Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs.

Required Experience / Certifications / Licensure:

• Licensed Physical Therapist in North Dakota.
• Current driver's license required and a car available to travel to patients' homes.
• Strongly preferred: 2 years previous experience as a Physical Therapist.
