The position is entirely remote, allowing for flexibility while maintaining a structured work schedule from Monday to Friday, 9:00 AM to 5:00 PM. Key Responsibilities\n

• Conduct thorough psychosocial assessments to ascertain patients' needs, strengths, and challenges, ensuring a complete understanding of their situation.

• Provide impactful counseling and health education tailored to individual patients, facilitating disease management through a combination of direct interaction and virtual support.

• Collaborate with a multidisciplinary healthcare team to develop and implement personalized care plans that focus on behavior modification strategies, coping methods, and effective lifestyle changes.

• Engage with patients through synchronous and asynchronous goal-setting exercises to empower them in managing both their chronic conditions and associated mental health needs.

• Perform regular outreach based on individual patient needs and preferences to promote client engagement and motivate behavior change.

• Accurately document patient interactions, progress notes, and care plans through our electronic health record (EHR) system, ensuring adherence to legal and ethical guidelines.

Qualifications\n
