We are committed to leadership in rehabilitation and deliver patient-centered care across inpatient, outpatient, and home health settings.Join Champion Rehab and make a meaningful impact—within our organization and in the lives of the clients and communities we serve.As an Occupational Therapist you'll:Direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when neededWork closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbiditiesApply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Certified Occupational Therapy Assistant, communicating with Physicians to ensure fluency of careComplete documentation which follows regulatory guidelines (state, federal)Qualifications:Graduate of an accredited Occupational Therapy programCurrently licensed to practice in State of MN.
