Medical Assistant

Description As a member of the Population Medicine team, you will work collaboratively within an interprofessional care team to provide proactive, person-centered support for patients living with chronic medical conditions. Using population health strategies and risk-based outreach to identify patients who may benefit from additional support, the CCM Medical Assistant helps ensure patients receive timely, coordinated, whole-person care that is responsive to their individual goals, preferences, medical complexity, and social needs.
