Care Manager Assistant - LPN

The Care Manager Assistant - LPN supports patient care coordination and service delivery in a post-acute setting, ensuring quality care through collaboration and compliance with laws and standards. Qualifications include a nursing diploma, active LPN license, and preferred clinical experience in post-acute care, case management, or discharge planning.
