Residential Case Manager: Client-Centered Recovery

The Providence Center seeks a Residential Case Manager to provide clinical case management within a residential setting for adults with complex behavioral health needs. You will coordinate with a multidisciplinary care team to assess, plan, implement and monitor individualized care plans and supports. Role emphasizes person-centered services, collaboration with families and community partners, and ongoing crisis intervention when needed.
