Ambulatory Clinical Pharmacist - Infectious Disease, Pulmonary Hypertension, & Neurology Facility: National Jewish Health - Main Campus Department: Pharmacy Schedule: Shift – Hours Full Time – Day Shift: Monday – Friday The Infectious Disease, Pulmonary Hypertension, & Neurology Ambulatory Clinical Pharmacist position offers a unique opportunity to provide advanced pharmacy services across multiple specialized clinics at National Jewish Health. This role supports patients with complex infectious, neurologic, and cardiopulmonary conditions requiring high-touch care coordination, specialized therapies, and close clinical monitoring. Working collaboratively with multidisciplinary teams, the pharmacist will play a key role in optimizing therapy outcomes, improving medication adherence, and enhancing patient access to life-changing treatments. Key Highlights Manage complex medication regimens including antimicrobials, pulmonary hypertension therapies, anticonvulsants, immunotherapies, and other specialty medications Collaborate closely with infectious disease providers, pulmonary hypertension specialists, and neurology teams to support evidence-based clinical decision-making Provide direct patient education, medication counseling, and longitudinal therapy monitoring to improve safety and adherence Assist with prior authorizations, financial assistance programs, and specialty pharmacy coordination for high-cost and limited-distribution therapies Monitor laboratory values, adverse effects, drug interactions, and therapeutic response across diverse disease states Serve as a clinical resource for providers and staff regarding emerging therapies, guideline updates, and medication management best practices The Ambulatory Clinical Pharmacist will serve as an integral member of the interdisciplinary care team, working collaboratively with providers, advanced practice providers, nursing staff, and other healthcare professionals to optimize medication therapy for patients with chronic conditions. This pharmacist will focus on guideline-directed therapy, chronic disease management, medication optimization, and patient education through a team-based care model. Evaluate, initiate, titrate, and monitor pharmacotherapy for patients in alignment with current clinical guidelines Practice under a Collaborative Practice Agreement in accordance with CRS 12‑280‑602 & Rule 17, collaborating with clinical providers to optimize medication regimens through: Initiation, adjustment, and titration of medications Identification and resolution of drug interactions, therapeutic duplications, and inappropriate therapies Ordering and interpretation of relevant laboratory tests Patient education and counseling Provide comprehensive medication therapy management services and chronic disease management to improve outcomes and support safe, effective, and individualized treatment plans Conduct detailed medication reconciliations and therapy reviews to identify potential drug-related problems and support best practices in prescribing Actively participate in daily huddles and multidisciplinary rounds to support real-time clinical decision making and care coordination Document all patient interactions, interventions, recommendations, and therapeutic decisions in the electronic medical record (EMR) in a timely and accurate manner Identify and resolve barriers to medication access and adherence, collaborating with patients, providers, and financial assistance resources to ensure treatment continuity Review complex medication orders to ensure accuracy, appropriate destination pharmacy, and communication with patients regarding changes Work closely with the Utilization Management and Prior Authorization teams to: Facilitate timely prior authorization approvals Assist with copay assistance and free drug programs Navigate specialty pharmacy processes and 340B opportunities Contribute to protocol development, clinical research initiatives, and implementation of evidence-based practice guidelines Provide medication-related education to providers, nursing staff, and clinic teams on emerging therapies, guideline changes, and best practices Participate in quality improvement projects and outcomes reporting that align with pharmacy, clinic, and institutional goals Meet or exceed established Key Performance Indicators (KPIs) and contribute to ongoing program evaluation and enhancement as defined by departmental and institutional goals Maintain working knowledge of 340B program requirements and support compliant day-to-day operations, including accurate patient eligibility determination and prevention of diversion and duplicate discounts in accordance with HRSA regulations and organizational policies Other Duties Be available as a resource for Introductory Pharmacy Practice Experience (IPPE) students and Advanced Pharmacy Practice Experience (APPE) students, coordinating with Pharmacy Operations Manager and pharmacy team to facilitate a positive learning environment Perform as a backup to main campus and off-site clinic locations as needed Accountability: Accepts full responsibility for self and contribution as a team member; taking action to achieve goals beyond what is required; being proactive Perseverance/Resourcefulness: Accesses and utilizes available resources inside and outside the company Supervisory or Managerial Responsibility : None Travel : Local travel may be required Minimum Qualifications Education: Doctor of Pharmacy (PharmD) from an ACPE-accredited institution required Work Experience: Postgraduate Year One (PGY1) residency required; PGY2 in ambulatory care or specialized area preferred; five years of recent and related experience in clinical pharmacy practice preferred Special Training, Certification or Licensure: Colorado Pharmacist License required; board certification preferred; ideal candidates may hold BCPS, BCACP, or specialty board certification such as BCCP, BCOP, BCIDP; BLS required Salary Range : $135,674.89 – $179,430.05 Benefits Comprehensive Medical Coverage: Multiple Cigna health plans for Colorado, regional office, and remote employees.
