Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides Person Centered care and services.Roles and Responsibilities:Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's /residents conditionsPractices ethically and professionally within their scope; advocates for patientMaintains clinical competence specific to their care area including organization required education within stated timeframeCoordinates care with interdisciplinary teamDocuments appropriately and thoroughly in EMR/Point Click CareKeeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observationsParticipates in unit councils and staff meetings.Weekend rotation requiredRequired Skills and Experience:Graduate of a Nursing ProgramCurrent NYS LPN LicenseCPR/BLSPreferred Skills and Experience:Prior patient care and customer service experiencePhysical Requirements:May require lifting (up to 50 lbs.), may require sitting or standing for long periods.
