Targeted Case Manager

As a Targeted Case Manager at Sevita, you’ll coordinate person-centered services for individuals with complex behavioral and medical needs in home and community settings. You will assess needs, develop care plans, link individuals and families to community resources, and monitor progress to ensure quality outcomes.
