Location: Shelbyville, TN 37160. Covering Bedford & Marshall Counties
Pay: $48.69/hour
Schedule: Monday-Friday, 8:00am-5:00pm. 40 hours/week guaranteed
On-call: No
Mileage Reimbursement: $0.43/mile
Job Overview
We are hiring a Home Health Registered Nurse (RN)to support home health patients in the Bedford & Marshall County area. You’ll assess patient needs, implement care plans, coordinate with providers, and ensure accurate clinical documentation.
Key Responsibilities
- Assess patient health status and develop/implement individualized care plans
- Provide nursing care and case management for chronic/acute conditions
- Coordinate with physicians and other healthcare professionals to support patient outcomes
- Monitor and manage medication administration processes and documentation
- Maintain accurate nursing notes and required clinical documentation
- Communicate changes in condition to the physician and follow care directives/orders
- Support training and guidance for staff as needed
Required Skills & Qualifications
- Active TN RN license (compact license accepted)
- Home Health experience
- Experience with HCHB (Homecare Homebase)and OASIS documentation
- Comfortable meeting productivity expectations (30 points/week )
Preferred Experience
- Strong home health documentation and care coordination background
- Comfort collaborating across disciplines (providers, care teams, discharge coordinators)
