This Per Diem / PRN Physical Therapist role provides the flexibility of mobile therapy and home-based care , allowing you to work independently while making a meaningful impact on patients in your local territory. Your Role: Home Health Physical Therapist As a Home Health Physical Therapist (PT) , you will deliver skilled physical therapy services in patients’ homes , assisted living facilities, and senior communities throughout Orange County and surrounding areas. Your work will focus on rehabilitation, fall prevention, mobility improvement, and restoring independence for patients recovering from injury, illness, surgery, or chronic conditions including dementia and geriatric health challenges .
