Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Current unencumbered Physical Therapy license for the state of Indiana
• Recent home care experience proficiency
• HomeCare HomeBase proficiency
• Understanding of Medicare guidelines for home care
• CPR certification
• Driver's license, vehicle insurance, and dependable vehicle

Assignment Details

• Shift hours: 8:00 AM - 4:30 PM
• Shift days: Monday-Friday
• Weekend requirement: Every 3rd Saturday
• Coverage area includes Wabash, Huntington, and Grant County, with the ability to support Blackford, Delaware, and Madison County as needed
• May require availability on observed holidays

Location Highlights

Marion, Indiana offers access to local culture and outdoor recreation in north central Indiana. Matter Park spans more than 100 acres along the Mississinewa River and includes trails, gardens, and open green space near downtown Marion. Indiana Wesleyan University is located in Marion, and Mississinewa Lake is about 25 miles away for boating, fishing, and camping. The city is within driving distance of Fort Wayne at about 50 miles northeast and Indianapolis at about 70 miles south, which supports both quieter living and access to larger metro amenities.
