Registered Nurse Care Manager - Inpatient

As an RN Care Manager at MLK Community Hospital, you will coordinate inpatient care for a diverse, high-acuity population in South Los Angeles. You’ll perform comprehensive assessments, lead discharge and transition planning, and collaborate closely with physicians, social workers, and allied health professionals to ensure safe, timely, and cost‑effective care. In this role, you will use clinical judgment and EHR tools to manage utilization, reduce readmissions, support families, and advance equity, quality, and culturally competent, patient‑first outcomes in a modern, mission‑driven hospital.
