Practical Nursing Specialist

Role Overview: Provides basic nursing care by assessing residents, communicating changes to the interdisciplinary team, and documenting all assessments accurately. Participates in care planning, updates care plans as needed, and implements nursing interventions in accordance with physician orders and resident needs. Ensures safe medication administration, treatments, and IV therapy (if certified). Uses critical thinking to address residents' condition changes and communicates effectively with physicians using SBARR. Monitors residents closely to ensure care plan adherence, identifies problem areas, and utilizes the QAPI process for resolution. Responds promptly to call lights and assists with activities of daily living (ADLs). Maintains professionalism during crises and stays current with nursing practices through ongoing education. Qualifications & Skills Licensed Practical Nurse (LPN) credential from an accredited school and current licensure in the state of practice CPR and AED certification; IV certification (or willingness to obtain within six months) Experience in long‑term care preferred Strong communication, time management, and organizational skills Physical ability to stand, walk, lift, push, and transfer residents and equipment safely Manual dexterity and visual acuity for accurate documentation and patient care Key Duties Assess residents upon admission and ongoing Communicate significant health changes to the care team Participate in and update care plans Administer medications and treatments safely and timely Respond to emergencies and monitor resident well‑being Complete required continuing education to stay current with best nursing practices #J-18808-Ljbffr DayToDay Health
