Role Description The Community Health Worker is a full-time, hybrid role based in Buffalo, NY, with some flexibility for work from home. In this role, the Community Health Worker supports individuals and families in high-needs communities by helping them navigate health and social services, including scheduling appointments, coordinating transportation, and connecting them to community resources. Daily responsibilities include conducting outreach, building trusting relationships, performing home or community visits, and providing education on preventive care, chronic disease management, and healthy lifestyle choices. The Community Health Worker will collaborate closely with clinical teams and social service providers, track member progress, and maintain accurate documentation in accordance with organizational standards. The role also involves advocating for members' needs, identifying barriers to care, and reporting emerging community trends to help inform CINQCARE's programs and services. Qualifications Strong interpersonal, communication, and relationship-building skills to engage respectfully with individuals, families, and community partners. Ability to conduct outreach, community engagement, and basic health education in a clear, culturally sensitive, and accessible manner. Skills in organizing tasks, managing time, and maintaining accurate records and documentation, including use of basic computer and mobile applications. Experience or demonstrated interest in working with high-needs, urban and rural communities, including understanding of social determinants of health. Ability to work both independently and as part of an interdisciplinary team, with sound judgment and problem-solving skills. High school diploma or equivalent required; additional training or certification as a Community Health Worker, peer advocate, or similar role is preferred. Fluency in English required; proficiency in additional languages commonly spoken in the Buffalo, NY area is a plus. Reliable transportation to perform community visits and attend meetings within Buffalo, NY and surrounding areas, as needed. #J-18808-Ljbffr
