Job Summary Responsible for assessing, implementing, and evaluating care for various patients within the facility. Reports directly to the Clinical Director of the unit. This position works in conjunction with other members of the nursing service, ancillary services, physicians, patients, and families to ensure quality of care across the continuum. Key Responsibilities Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum. Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis. Continually utilizes the nursing process to evaluate patient's progress toward goals and applies appropriate interventions to ensure effective outcomes. Employs strategies to promote health and a safe and healthy environment. The responsibilities listed are a general overview of the position and additional duties may be assigned. Technical Capabilities Nursing Patient Assessment & Evaluation (Novice): Demonstrates sufficient fundamental proficiency to assess and evaluate patients in moderate‑difficulty practical applications, conduct primary care interviews and physical examinations, recognize problems, ask questions and contact primary care providers and other healthcare team members, and gather clinical data to aid in referral and treatment pathways. Nursing Patient Education (Novice): Provides patient education in moderate‑difficulty practical applications with sufficient fundamental proficiency. Care Planning (Novice): Anticipates resource needs, identifies distinct tasks, sets priorities, schedules activities, meets deadlines, and organizes work within direct responsibility, handling multiple assignments simultaneously, adapting to changes with minimal disruption, and following up to resolve problems and issues. Evidence‑Based Practice (Novice): Searches for evidence to answer clinical questions under the direction of others while completing assignments.
