Role: Assists in the creation, planning and implementation of the Connections program - the daily activities for our residents. You will ensure the maximum levels of resident leisure contentment, as well as physical, mental, and psychological well being. Responsibilities: Entering resident information in multiple systems Attending care plan meetings with families and residents Collecting resident profiles Assist in the planning, developing, organizing, implementing, and evaluating of the Connections activity program for the community. Requirements: High School Diploma (Required) Must be 18 years or older Memory Care experience (Preferred) Previous experience in healthcare or service related setting (Preferred) Certification in CPR and First Aid (Preferred) Equal Opportunity Employer
