Job Description We are seeking a dedicated Clinical Case Manager to join our team. The ideal candidate will have a strong background in nursing and be passionate about coordinating comprehensive care plans for our patients. RN Case Manager Key Responsibilities Develop and implement individualized care plans, ensuring patient needs are met and goals are achieved. Collaborate with healthcare providers, including physicians and social workers, to ensure comprehensive patient care. Provide patient education and support to promote behavioral health and self-care. Monitor and report patient outcomes, identifying areas for improvement and developing strategies for improvement. Maintain accurate and detailed patient records, adhering to medical records and HIPAA regulations. RN Case Manager Qualifications Current RN license in New York. Experience in homecare preferred. Excellent communication and collaboration skills. Strong analytical and problem‑solving skills. Ability to work effectively in a fast‑paced environment and prioritize tasks. Knowledge of medical records and HIPAA regulations.
