Licensed Practical NurseCambridge Health Alliance's Program of All-Inclusive Care for the Elderly (PACE) is a nationally recognized model of care that serves adults aged 55 and older living with chronic medical and/or mental health challenges.At CHA PACE, our mission is to support our program participants in achieving the highest level of independence within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment.We are seeking a highly skilled and dedicated Licensed Practical Nurse (LPN) passionate about making a difference in the lives of senior adults to join our dynamic interdisciplinary team at CHA PACE. In this rewarding role, you will provide direct nursing care to elderly participants both in their homes and in our PACE day clinic, ensuring continuity and quality of care across all settings.• Deliver hands-on nursing care in participants' homes and at PACE clinic settings, in collaboration with the RN and PACE interdisciplinary team (IDT)• Assist in the assessment, planning, and implementation of individualized care plans• Utilize evidence-based practices to support participants' physical and emotional well-being• Identify and communicate significant changes in participants' conditions to the RN and IDT• Support clinic triage and provide cross-coverage as needed to ensure uninterrupted care delivery• Maintain accurate and timely documentation of care and services providedGraduation from an accredited licensed practical school of nursingCurrent or conditional Massachusetts LPN license1 year of experience in geriatrics preferred1-2 years of home care experience preferredMaintains current Basic Life Support (BLS) certificationKnowledge of a second language preferredMust have a car to travel to participants' homesIn keeping with federal, state and local laws, Cambridge Health Alliance (CHA) policy forbids employees and associates to discriminate against anyone based on race, religion, color, gender, age, marital status, national origin, sexual orientation, gender identity, veteran status, disability or any other characteristic protected by law.
