Location: This position is based in Ft Myers, FL, serving patients in Lee County Your Role as a Licensed Practical Nurse (LPN): Deliver high-quality nursing care to patients in their homes based on individualized care plans. Perform a variety of tasks, including: Acute and chronic disease management. Medication management and administration. Assessing patient conditions and reporting changes. Developing tailored, goal-oriented treatment plans. Post-surgical rehabilitation. Educating patients and caregivers to support recovery and independence. Pain management and wound care. Qualifications: Unencumbered Florida LPN License (required). Valid driver’s license, auto insurance, and reliable transportation (required). IV Certification (required).
