Job Summary:
Registered Nurse (RN) specializes in providing skilled home health care to geriatric patients. This role involves delivering comprehensive clinical assessments and treatments within patients' homes, ensuring quality care and regulatory compliance.
Responsibilities:
- Conduct and document start-of-care assessments, routine visits, recertifications, and transfers in accordance with agency protocols.
- Administer medications, IV therapy, and manage wound care, pressure ulcers, and wound VACs.
- Manage feeding tubes, G-tubes, ostomies, and urinary catheters with precision and safety.
- Perform clinical documentation within 24-48 hours, including OASIS submissions and visit notes.
- Collaborate with interdisciplinary teams to develop and adjust patient care plans.
- Maintain compliance with all regulatory standards and safety protocols during home visits.
Qualifications:
- Require 2-3 years of recent skilled home care experience, including clinical assessments and documentation.
- Must possess OASIS documentation experience and proficiency with WellSky/Kinnser/charting systems.
- Hold a Maryland or compact RN license, with BLS certification.
- Demonstrate competence in wound, IV, medication, and catheter management.
- Have a valid driver’s license and reliable vehicle for local travel within Baltimore County and support for Baltimore City.
- Able to work Monday–Friday, 8:00 a.m. to 4:30 p.m., with optional on-call coverage as needed.
