Advanced Practice Clinician

Location: Bronx, NY

Employment Type: Per Diem / Contract
Assignment Length: Approximately 90 days with potential for extension
Schedule: Flexible days, evenings, and weekends

Position Summary

We are seeking an experienced Nurse Practitioner or Physician Assistant to conduct in-home preventative health assessments for health plan members. Providers will travel throughout their assigned territory and complete comprehensive assessments within members' residences.

This position offers flexible scheduling and is ideal for providers with primary care, home health, or previous in-home assessment experience.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Review medical history, medications, current health conditions, and preventative care needs.
• Accurately document assessment findings using an electronic medical record system.
• Submit completed documentation within required timeframes.
• Respond to documentation or charting questions as needed.
• Maintain consistent communication with clinical leadership.
• Complete required training and attend applicable meetings.
• Maintain patient confidentiality and comply with HIPAA requirements.
• Provide a minimum of approximately 30 completed visits per month .
• Travel throughout the assigned service area to members' residences.

Qualifications

• Active Nurse Practitioner or Physician Assistant license in New York.
• NP candidates must have a Master's or Doctorate degree from an accredited Nurse Practitioner program.
• PA candidates must have a Master's degree from an accredited Physician Assistant program.
• Appropriate national board certification.
• Minimum of approximately 3,600 hours of applicable clinical experience .
• Recent primary care and/or home health experience strongly preferred .
• Previous in-home assessment experience highly preferred.
• Current Medicaid enrollment/Medicaid ID in New York strongly preferred.
• Current Medicare enrollment credentials as applicable.
• Current BLS certification.
• Valid driver's license and appropriate automobile insurance.
• Reliable transportation and ability to travel within the assigned territory.

Skills and Competencies

• Strong clinical assessment and documentation skills.
• Proficiency with electronic medical records and general computer systems.
• Strong verbal and written communication skills.
• Ability to work independently and manage a flexible schedule.
• Strong attention to detail and ability to complete documentation within established deadlines.
• Ability to adapt to different home environments and patient needs.
• Understanding of Medicare, Medicaid, preventative care, and risk-adjustment concepts preferred.
• Ability to maintain professionalism and patient confidentiality at all times.

Physical Requirements

• Comfortable entering members' personal residences.
• Ability to lift approximately 20 pounds.
• Ability to bend, kneel, or remain in various positions as needed while completing assessments.

Appointments generally last approximately 60-75 minutes , and providers may schedule visits during daytime, evening, or weekend hours based on availability.
