Description

• Conduct thorough evaluations of patients in inpatient or outpatient settings and craft personalized treatment plans
• Implement specialized physical therapy interventions focusing on mobility, strength, balance, and overall functional improvement
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Educate patients and their families on therapeutic objectives, discharge processes, and at-home exercise routines
• Accurately document patient evaluations, treatments, and progress in compliance with both facility and regulatory standards
• Participate in interdisciplinary conferences and care team rounds to ensure seamless care coordination

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Any specialized certifications (such as neurological or geriatric) are a plus but not mandatory

Skills

• Prior experience in a clinical setting is highly preferred
