NOW HIRING: SOCIAL WORKER

We are seeking a compassionate, dedicated, and professional Social Worker to join our healthcare team and advocate for the emotional, social, and overall well-being of our residents.

Position: Social Worker
Location: Tinton Falls, NJ 07724

Responsibilities:

• Provide psychosocial support and counseling to residents and their families

• Complete social service assessments and maintain accurate documentation

• Develop and implement individualized care plans

• Assist residents and families with adjustment to the facility and changes in health or circumstances

• Participate in care plan meetings and interdisciplinary team discussions

• Advocate for residents' rights, needs, and preferences

• Coordinate discharge planning and appropriate community resources

• Communicate with families, healthcare providers, and outside agencies

• Address resident concerns and help resolve social and emotional issues

• Provide support during transitions of care

• Maintain confidentiality and comply with facility policies and applicable regulations

Qualifications:

• Bachelor's degree in Social Work or related field required

• MSW preferred

• LSW/LCSW preferred or required based on applicable regulations

• Previous experience in skilled nursing, long-term care, healthcare, or geriatric services preferred

• Excellent communication and interpersonal skills

• Compassionate, patient, and professional approach

• Strong organizational and documentation skills

• Ability to work effectively as part of an interdisciplinary team
