You'll partner closely with patients, families, providers, and the interdisciplinary team to develop individualized plans of care that support comfort, dignity, and quality of life.What You'll DoAssess patient needs and develop individualized care plansProvide hands-on nursing care, education, and emotional support to patients and familiesCollaborate with physicians and interdisciplinary team members to coordinate careDocument patient progress and communicate changes in conditionParticipate in care conferences, team initiatives, and ongoing educationSupervise and support LPNs and LNAs assigned to patient careTravel throughout the service area to provide in-home patient visitsRequirementsGraduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).One to two years of recent general medical/surgical or skilled/acute care nursing experience.
