Remote RN Case Manager - Care Coordinator

CVS Health is seeking an experienced Case Manager RN for a full-time remote role. You will assess and coordinate care for members, supporting their short- and long-term wellness from your home office. Responsibilities include evaluating member needs, leveraging clinical tools, and collaborating with the care team to reduce risks and barriers while maintaining HIPAA compliance. 40 hours/week, Mon-Fri, in residence time zone. #J-18808-Ljbffr
