Don't wait-join our team and help move health and hope 4ward! #4CHealth #MentalHealth #CareersWithPurpose Position Summary: To monitor, track, and assist with implementation of Behavioral Physical Health Coordination (BPHC) services at 4C Health. This will include data collection, and ongoing monitoring, tracking and evaluation which will ensure implementation of quality practices and procedures of the Center as it relates specifically to BPHC. To complete all applications for consumers who qualify for this service. To provide direct BPHC services with office-based linkage and case management services relative to BPHC. To be involved in monitoring and support of direct staff who provide BPHC and collaborate with the individuals involved in BPHC services. This position is responsible for promoting mental health recovery through a person-centered approach focusing on independence and integration. This is accomplished through delivery of appropriate mental health assessment, treatment planning, orchestrating case management with internal and external treatment team members and intervention for clients impacted by emotional/behavioral impairment and serious, persistent mental illness who are targeted for mental health services by the Division of Mental Health. Work is very broad in scope and requires a high level of professionalism. ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Complete comprehensive assessment as it relates to BPHC application, including Adult Strengths and Needs Assessment. Also may include intake, evaluating a client's ability to meet the admission criteria for that particular department, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse and abuse/neglect.
- Must develop objectives related to BPHC that are individualized, time-limited, and behaviorally stated that target the client's challenges. They must communicate these objectives with direct services providers involved in the client's care.
- Documentation must be appropriate, accurately reflecting the progress of the client and the treatment planning goals. Service notes must be completed for each client that accurately specifies the service content and the individual client's participation and reaction to content. All documentation is to be entered within 24 hours of the service provided, in accordance with agency policy.
- Provide progress reports to referral sources as needed for each individual client.
- Completes all necessary and required monthly progress reports and ensures appropriate care coordination.
- Periodically communicate with the referring agency, at least monthly, to provide progress updates and obtain relevant collateral information from the referral source and other external treatment team members.
- Participate, coordinate, and engage in treatment team meetings with others involved in the client treatment plan (internally and externally).
- Provide skills training and development as a primary intervention to all clients on caseload making sure that skills training is based on each individual client and their vision for recovery.
- Provide direct service case management and skill building intervention for referred clients and their families addressing goals and objectives as outlined on the treatment plan.
- Provide clinical client contact hours based on billable counts of service, FTE, and area of services (school based, community based, ACT, group home etc.) as determined by agency policy.
- Meet UE (Unique encounters) as set by the department/manager.
- Work evenings and weekends as assigned or necessary.
- Completion of all tasks from admission to discharge and aftercare (including court documentation) for the Inpatient Psychiatric Unit case management team.
- Meet with assigned clients and/or families as frequently as outlined in the treatment plan.
- Transport clients to appointments or other destinations (relevant to the treatment plan).
- Assist clients in the development of natural support systems to foster independence as much as possible.
- Implement and utilize 4C Health approved curriculums and trained evidence-based practices for intervention
- Participate in continuing education pertaining to case management, mental health, addictions, or other areas applicable to services provided and populations served.
- Upon meeting qualifications, it may be requested to participate in Mobile Crisis response or on-call services.
- Complete required supervision (individual and group) per Center policy.
- Abide by all Center policies and procedures.
- Maintain positive working relations and rapport with the communities served.
- Perform other duties and projects as assigned.
- Must maintain a high level of integrity, discretion, and a confidential approach regarding numerous sensitive matters related to the position. Information such as contract agreement consumer files, etc must not be disclosed except as necessary in connection with the performance of work.
- Must be able to manipulate databases to analyze, monitor, and evaluate all relevant Center data pertaining to BPHC.
- Monitor and track DARMHA website daily and respond immediately to any information provided on that database regarding BPHC.
- Complete all applications on an ongoing basis for anyone in the Center who qualifies for BPHC.
- Monitor new and existing clients who might become eligible for BPHC and respond by linking them to the service with application completion and follow-up.
- Utilize as many BPHC units as possible from the office with case management and linkage type services, working closely with applicable service providers (such as Med Clinic, Comm.-Based, Outpatient).
- Work closely with the BPHC team to ensure current information is being applied to all duties.
- Assist with training of all staff that need to stay current on relevant information of this program, including implementation of BPHC Units.
- Regularly monitor agency documentation and use of BPHC units to ensure compliance with BPHC service standards.
- Assist with documents related to BPHC for review/approval by quality committee, peer review committee & BOD, as applicable.
- Assist in preparing for external review regulators related to BPHC.
- Pursue methods to ensure receipt of relevant data, as required for accurate trending of BPHC information.
- Analyze key plan metrics specific to BPHC, such as operational performance, reporting mechanisms, eligibility criteria, and tracking of BPHC Units to enable the development of sound and valid recommendations regarding and prioritization of clinical and service improvement initiatives relative to BPHC, working closely with supervisor and BPHC committee.
- Acts as knowledge expert for BPHC, educating staff of other functional areas regarding BPHC process and implementation.
- Makes recommendations on matters of policy/procedure which are in line with current BPHC standards.
- Four-day (32 hour) work week
- Make up to an additional $525 per week by participating in Mobile Crisis On-Call opportunities in eligible counties
- Eligible for $150/month ($1,800 annually ) in student loan repayment, available at hire
- Earn $750 for every person you refer who is hired and remains employed for 6 months and receive an additional $500 every year as long as you both remain employed

- Recovery Facilitators require a minimum a High School graduate or GED.
- An Associates or Bachelor degree or further education in social services, psychology or mental health preferred but not required.
- If highest degree is High School Diploma, then candidate must have a minimum of 2 years of qualified work experience in behavioral health, have completed a technical certificate in behavioral health, human services, or other relevant social services or have 2 years (60 credit hours) of coursework.
- If working with families and individuals experiencing substance use disorder, the additional certification (such as through ICAADA) is preferred but not required. Peer Certification by The State of Indiana or through ICAADA is included here.
- If working in community based with DCS referrals, applicant must meet DCS service standards specific expectations for education and qualifications.
