Home Health RN Case ManagerStaffing Care ProStaffing Care Pro is seeking a dedicated and compassionate Home Health RN Case Manager to join our growing care team. This role is ideal for a Registered Nurse who values autonomy, meaningful patient relationships, and a consistent weekday schedule while delivering high-quality care in the home setting.As a Home Health RN Case Manager, you will play a key role in coordinating patient care, supporting caregivers, and ensuring compliance with clinical and regulatory standards. You will work directly with patients in their homes, collaborate with providers, and help guide care plans that promote safety, independence, and improved health outcomes.Key ResponsibilitiesConduct comprehensive in-home patient assessments, including physical evaluations and ongoing reassessmentsDevelop, implement, and manage individualized plans of care based on patient needsSupervise and provide clinical oversight to home health aides, LPNs, and other care team membersEducate patients and families on care plans, disease management, medications, wound care, and other home health needsCommunicate changes in patient condition to physicians and interdisciplinary care teamsComplete accurate and timely documentation, including OASIS assessments and required clinical recordsPerform weekly chart audits to ensure accuracy, quality, and compliance with regulationsSupport continuity of care and help reduce hospital readmissions through proactive patient managementParticipate in team meetings, training, and ongoing quality improvement initiativesPerform additional duties as assigned to support patient care and agency operationsQualificationsActive, unrestricted Indiana Registered Nurse (RN) licenseMinimum of one (1) year of RN experience in home health; case management experience preferredStrong knowledge of OASIS, Medicare guidelines, and home health regulationsExcellent assessment, critical-thinking, and problem-solving skillsAbility to work independently while managing a patient caseloadEffective communication skills with patients, families, caregivers, and providersValid driver's license, reliable transportation, and ability to travel within the assigned territory (approximately 25%)Proficiency with technology and electronic documentation systemsMust have a personal computer and high-speed internet; Microsoft Teams used for regular communicationSchedule & CompensationFull-time | Monday–Friday | Day shiftOn-call as neededSalary range: $70,000–$80,000 annually, based on experienceBenefitsFlexible schedulingHealth insurancePaid time offTuition reimbursement potentialSupportive leadership and collaborative work environmentEducation & CertificationsBachelor 's degree in Nursing preferredCurrent BLS/CPR certificationU.S. work authorization required
