Medical Director - Hospice & Palliative Care opening in Hartford, CT

Seeking board certified Medical Director – Hospice and Palliative Care to join team in Hartford, CT. Lead a team of hospice physicians and per diem APRNs caring for patients across Connecticut. Responsibilities Participate in the decision-making process in relation to admission to the hospice (homecare) or inpatient setting Participating as a member of the Interdisciplinary Team in the development of individualized care plans Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit Consult with attending physicians regarding pain and symptom management Coordinate efforts with the attending physician to provide continuity of care Develop and monitor the medical component of the hospice program Participate in the development, coordination and implementation of hospice services and policies Participating in the development of staff and volunteer training Conduct face-to-face visits for hospice admissions and recertification when appropriate as required by CMS.
