Your Role

Were seeking a compassionate, experienced Home Health Physical Therapist to join our high-performing team. The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. Youll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy.

What Youll Do

Conduct OASIS assessments and initial start-of-care visits to establish individualized plans of care
Evaluate functional limitations and develop personalized therapy goals and interventions
Guide patients through therapeutic exercises and mobility programs to maximize independence
Educate patients and caregivers on safe home exercise programs and fall-prevention techniques
Collaborate closely with physicians, nurses, and other care-team members to coordinate care
Monitor patient progress, update care plans, and maintain thorough, compliant documentation
Ensure compliance with all Medicare, CMS, and home health standards

What Were Looking For

Current Florida Physical Therapist (PT) license (required)
Previous home health experience is strongly preferred but not required
Excellent communication, time management, and documentation skills
A compassionate, dependable, and professional attitude
A team-oriented mindset and a genuine passion for making a difference in patients lives
