Position Summary
Paragon Behavioral Health Connections is seeking a Bachelor-Level Care Manager to provide intensive community-based care management services within the Community Access & Partnership (CAP) Program .
The CAP program serves adults with Serious Mental Illness (SMI) and Severe and Persistent Mental Illness (SPMI) whose psychiatric, functional, and social needs create significant barriers to engaging in traditional outpatient services. Utilizing assertive community outreach and a multidisciplinary team approach, the Care Manager provides intensive care coordination, resource navigation, housing support, community linkage, benefits assistance, and skill development services within clients' homes and natural environments.
The Care Manager plays a critical role in helping clients achieve psychiatric stability, maintain housing, reduce crisis utilization, improve independent living skills, and successfully transition toward less intensive levels of care.
This position requires extensive field-based work, community outreach, and collaboration with internal and external providers throughout the Denver Metro area and surrounding communities.
Key Responsibilities
Intensive Care Management & Community Support
- Provide intensive, community-based care management services in clients' homes and natural environments.
- Conduct outreach and engagement activities with individuals who experience barriers to treatment participation.
- Coordinate services across behavioral health, medical, housing, employment, vocational, and social service systems.
- Assist clients in identifying strengths, needs, goals, and barriers to recovery.
- Assist with obtaining identification documents, benefits, transportation resources, healthcare access, and community supports.
Behavioral Health & Recovery Support
- Assist clients in practicing and applying coping skills, wellness strategies, and recovery supports within real-world environments.
- Provide psychoeducation regarding mental health recovery, medication adherence, self-advocacy, and available community resources.
- Support clients with appointment attendance and coordination of care across providers.
- Utilize motivational interviewing and trauma-informed approaches to increase engagement and promote self-determination.
- Collaborate closely with therapists, psychiatric providers, peer specialists, and supported employment staff.
Housing Stability & Community Integration
- Assist clients experiencing homelessness, housing instability, or risk of eviction.
- Support applications for subsidized housing, supportive housing, vouchers, Medicaid-funded housing supports, and other community resources.
- Coordinate with housing authorities, landlords, shelters, residential providers, and community partners.
- Assist clients in overcoming barriers to housing including required documentation, benefits applications, budgeting, and community referrals.
- Support clients in maintaining housing stability through advocacy, coordination, and skill-building interventions.
Healthcare & Benefits Coordination
- Coordinate access to primary care, specialty medical care, psychiatric care, dental care, and community supports.
- Assist clients with Medicaid, Medicare, Social Security, SNAP, Long-Term Care Medicaid Waivers, and other public benefit applications.
- Support discharge planning following hospitalization, residential treatment, crisis stabilization, or incarceration.
- Monitor and address barriers that could impact treatment engagement or clinical stability.
Documentation & Compliance
- Complete all required documentation in accordance with agency, Medicaid, and regulatory requirements.
- Maintain compliance with HIPAA, confidentiality standards, and professional boundaries.
Transition & Discharge Planning
- Assist clients in developing transition and discharge plans that promote long-term stability.
- Facilitate successful linkage to outpatient services, community supports, healthcare providers, and recovery resources.
- Coordinate warm handoffs to lower levels of care as clients progress through the CAP program.
- Support clients in maintaining gains achieved during program participation.
Required Knowledge & Skills
- Knowledge of Serious Mental Illness (SMI), Severe and Persistent Mental Illness (SPMI), and co-occurring mental health and substance use disorders.
- Understanding of recovery-oriented, trauma-informed, person-centered care approaches.
- Knowledge of Medicaid, Social Security, housing resources, waiver programs, and community support systems.
- Strong crisis de-escalation, engagement, and rapport-building skills.
- Excellent written, verbal, and interpersonal communication skills.
- Strong organizational and time-management skills.
- Understanding of HIPAA, confidentiality requirements, and ethical service delivery.
