Physical Therapist needed for a Skilled Nursing Facility in Dallastown, Pennsylvania. This travel assignment starts on 08/03/2026 and runs for 13 weeks. Responsibilities
- Conduct PT evaluations for residents and develop individualized treatment plans.
- Implement and adjust therapeutic activities focusing on mobility, strength, balance, and gait training.
- Document progress, re-evaluate as needed, and communicate with the interdisciplinary care team.
- Assist with discharge planning and coordinating follow-up care, including home exercise programs.
- Ensure patient safety, maintain appropriate documentation, and adhere to facility policies and infection control practices.

Requirements
- Active Physical Therapist license in Pennsylvania (or eligibility to practice in PA)
- Experience in geriatric rehabilitation or skilled nursing settings preferred
- Ability to work in a long-term care environment and assist with mobility-related tasks
- Strong documentation and communication skills
- Willingness to work a travel assignment with 5x8 day shifts (08:00–16:30)
