Per Diem Occupational Therapist (OT) - Join Our Team at Hope Creek Nursing and Rehabilitation Center!
At Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois, we are seeking a dedicated and compassionate Per Diem Occupational Therapist (OT) to join our interdisciplinary team. As a vital member of our staff, you will play a crucial role in helping our patients achieve their highest level of independence and well-being.
About the Role:
As a Per Diem Occupational Therapist (OT), you will work closely with our skilled nursing team to provide high-quality care to our residents. Your responsibilities will include:
Assessing and evaluating patients' physical, cognitive, and emotional abilities
Developing and implementing individualized occupational therapy plans to address patients' needs
Providing hands-on therapy and treatment to improve patients' functioning and independence
Collaborating with multidisciplinary team members to ensure comprehensive care
Maintaining accurate and up-to-date records and documentation
