Key Responsibilities\n

• Act as the primary liaison between clients, their families, and outpatient providers to ensure coordinated care and effective communication.

• Utilize advanced clinical skills to interact with insurance companies for pre-certification processes and facilitate utilization management.

• Engage actively in discharge and aftercare planning, contributing to comprehensive treatment plans that support client needs beyond their immediate care.

• Provide therapeutic meal support, modeling behaviors that foster a healthy relationship with food, and taking into account cultural and personal preferences.

• Deliver empathetic care tailored to each client's unique circumstances and identities, ensuring that all interventions are affirming and culturally relevant.

Patient Population / Client Focus\n

The primary patient population for this role includes clients within the hospice setting—individuals facing terminal illnesses and their families. You will work closely with older adults, addressing both psychological and social needs as they navigate challenging transitions. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Possession of a valid Licensed Clinical Social Worker (LCSW) license in Pennsylvania.

Compensation and Benefits\n

This role offers a competitive salary ranging from $90,000 to $105,000 per year, commensurate with experience.
