Key Responsibilities\n

• Conduct comprehensive assessments and develop tailored treatment plans that align with each client’s unique needs and goals.

• Facilitate individual, group, and family counseling sessions utilizing evidence-based therapeutic techniques to address a range of mental health disorders.

• Regularly monitor and evaluate client progress toward established treatment goals, adjusting plans as necessary to ensure effective outcomes.

• Collaborate closely with psychiatrists, psychologists, and other healthcare professionals to deliver integrated and holistic care.

• Provide crisis intervention services for clients experiencing acute distress, ensuring safety and immediate support.

• Maintain accurate and timely records of client interactions and treatment services provided, adhering to confidentiality and ethical standards.

Patient Population / Client Focus\n

The primary client base includes individuals dealing with mental health challenges and substance abuse issues, as well as those facing significant life transitions such as relationship conflicts, divorce, grief, and bereavement. You will work closely with diverse populations, crafting personalized care strategies to support their paths to recovery and emotional health.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited program is required.

• A valid state license to practice clinical social work.

Preferred Experience\n

• At least two years of prior experience as a Licensed Clinical Social Worker, particularly in a hospice or healthcare setting, is preferred.
