Physical Therapist

As a Physical Therapist, youll contribute to Elara Carings success by providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists. You will design and implement care plans based on thorough assessments, incorporating participation by family members or caregivers when appropriate.

• Promotes Elara Carings philosophy, mission statement, and policies to ensure quality of care.
• Provides direct patient care and adjusts care plans to ensure optimal outcomes.
• Documents evaluations, treatment goals, and plans, maintaining appropriate records and signatures.
• Directs and provides leadership to staff to achieve patient goals.
• Conducts therapy practice within defined standards of care.
• Provides direction and leadership to staff and promotes physical therapy in the community.
• Work may involve physically demanding tasks and potentially irregular hours.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved program.
• Current, unrestricted PT license for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Ability and willingness to travel within branch coverage area (up to 50%).
• Valid drivers license, dependable vehicle, and current auto insurance.
