Community Health LPN

The Community Health LPN provides and documents nursing care of the sick and injured at home. The LPN intervenes to promote, maintain or restore health to prevent illness and to effect rehabilitation. The LPN collaborates with other healthcare providers, professionals and community representatives to coordinate care at home. The LPN evaluates the individual's and families' response to interventions. Ability to function independently under general guidance of the Director and follows the guidelines and protocols and/or policies and protocols specific to each unit. Duties require independent action and judgment. Utilizes time management, communication, planning, delegating, and problem solving in providing care. Must possess and demonstrate critical thinking skills. Must maintain emotional stability in emergency settings.

• History of dependability in meeting demands of committed work schedule.

CLIENT PREFERS 45 MIN OR LESS COMMUTE FROM TOR OFFICE ( 165 Tor Ct, Pittsfield, MA 01201)

• One year acute nursing care [required], hospital based preferred.
• Experience in home health [Required]
• Certified Home Health Required Including Proficiency with OASIS [Highly Preferred]
• Infusion Therapy Experience [Preferred]
• Demonstrated knowledge of the principles and practices of nursing.
• Knowledge of and ability to apply community health principles and practice.
• Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
• Experience with self-scheduling of caseload preferred.
• History of flexibility in assuming varied assignments.
• Current Driver License (and insurance) [Required]
• In order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines.
• Traveler candidate must have their own vehicle. This is a firm requirement.
• Traveler perm address must be included in profile
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer/at interview. Please obtain this information prior to submittal. Requests after offer may lead to rescinded offer
