Your Role: Bilingual Community Health Worker (CHW)

Location: Anaheim, CA

Pay: $31.00/hourly

Your Toolkit Should Include:

• High school diploma or equivalent; related Associate's or Bachelor's degree preferred
• Bilingual in English and Spanish required
• Experience in community outreach, family engagement, peer support, or case navigation
• Strong communication skills and ability to work with diverse families and communities
• Basic computer and documentation skills
• Knowledge of and connection to the Anaheim community preferred

Your Superpowers Include:

• Connecting families with health, behavioral health, and community resources
• Providing outreach, health education, and family support
• Helping families navigate referrals, appointments, paperwork, transportation, and other barriers to care
• Building trusted relationships with families, residents, and community partners
• Participating in community events, workshops, playgroups, and outreach activities
• Tracking outreach, referrals, and program activities
• Supporting culturally responsive, family-centered services for families with children prenatal through age 5
